- Down load and complete: IMA Volunteer Record, Disaster Code of Ethics and OB/IMA Medical
Dental Release. These forms can be e-mailed to the NOLA Medical Clinic

- Doctors and Nurses complete additional forms as specified below for temporary Louisana license
Physicians! Volunteer Forms Required to Work in Louisiana
To work as a volunteer in the NOLA Medical Clinic you must apply for a temporary
Louisiana Emergency Medical Permit.
- Download the State of Louisiana Emergency Medical Permit and State of Louisiana
Volunteer Agreement.
- Fax completed forms, a copy of your medical license and a picture ID to Erin Brewer, M.D.,
